Getting Around - North Hollywood, Van Nuys, Pacoima -- ???


Valley Interfaith Council Transportation Services



The para-transit transportation services are provided through the Multipurpose Senior Centers as a personalized door-to-door service offering a shared ride for the elderly 65 years of age or older and persons with disabilities.
  • A doctor must certify the need for door to door assistance.
  • Van trips may be up to 30 minutes from the senior's home

Service Information for Northwest Valley Adult Day Support Center

NORTHWEST VALLEY ADULT DAY SUPPORT CENTER is an adult day care facility.

This is a daytime program designed to engage seniors with health or other social need during the day. Typically this would be a place where the senior would be able to receive care if they are physically, mentally, or functionally impaired. Adult day care facilities often provide stimulating activities, companionship or simply supervision, if necessary.

NORTHWEST VALLEY ADULT DAY SUPPORT CENTER is an in-home care provider. In Home care could be either medical ("skilled") or non-medical ("custodial") support services delivered at the home of the senior.

"Skilled" service providers who usually provide various medical care needs and "custodial" care providers who provide services for daily living such as bathing, dressing, and meal preparation but may also extend to assistance with transportation, paying bills, making appointments, and simply being there to provide companionship and emotional support.

Home Care services are generally available 24 hours a day, seven days a week and are sometimes paid for directly by the client or through a variety of public and private funding sources such as Medicare and/or Medicaid.

Make sure you know your loved one's Medicare and Medicaid eligibility, and understand how you can use tools like long term care insurance to pay for in home care.

We support individuals and families by focusing on the issues that impact our elders and others who may not find it so easy to navigate the complexities of today's world.

With many pieces of info on-line, such info may not be easily found,accessed, or implemented by our elders. In many families, (grown) children live out of town and away from their parents and are so deluged with their own work, home, and kids' requirements that they cannot give the time they wish to their own parents and older loved ones... the time necessary to ensure that everybody knows what is going on in the elder's community and knowing all that he or she can take advantage of, and thereby continue to participate in and enjoy life.


We can help.

We have many services that deliver tailored information and knowledge to you or your dear one that can encourage community engagement and participation simply through clear, lovingly delivered information and explanation.

A small sampling of Serving Wisdom's services include:

a) Personal & Financial Record Keeping - review and maintain records, bill payments, other data via online computer services such that appropriate individuals can review the data as required (from anywhere)

b) Organization & Storage of Yesterday's Records - set up files for data and info needed now and save or toss the older data; maintain a clear set of records of what is where

c) Emergency Services (Customized) -- create emergency service listings and other information specific to a location to ensure your family member or friend knows how to seek and find help in a hurry

d) Upcoming Events (Customized) -- provide monthly a calendar or schedule of events specific to the individual. From gallery openings and museums to gun shows and bowling, we can find the events -- and how to get there and participate. Whatever can be unearthed and communicated to support your loved one in enjoying his or her life is the ongoing goal.

e) ad hoc Research, Letter Writing, Troubleshooting -- help to address new inquiries or come to closure on older issues -- we provide the best information for the best decision-making specific to your family's needs.

f) Community Resources -- a monthly publication highlighting new info from the Government and other community resources. For example, take advantage of free fire department check-out of smoke alarms; find out where the next flu clinic will be; get that new report on scientific and medical research as well as helpful info on changing laws or regulations.

Our monthly publication includes a variety of news, research, and ideas that you can parlay into help for your family and their place in the community...with special and specific insights as you identify and request.
